Working Hours: 8:00am-5:00pm
Summary:
Responsible for the direct social work case management services to PACE participants. Provides social service support to include participant screening and assessments, case management, counseling and referral.
Other information:
● Master's degree required
● LMSW or LCSW required
● Two years experience in healthcare preferred
● Experience working with the frail and elderly population preferred
● Excellent customer service skills
● Excellent interpersonal communication skills
This job will be authorized 80.00 hours bi-weekly.
